Doctor won't let you name shelters to trash them. I think Journeyman, and presumably other name-a-card effects have the same problem.

Yes. There is currently no way for you to select cards that don't have a spot in the supply, or near the supply. This is high on their priority list and I think we will see that feature in the near future.
Split piles and Knights may have the same issue.
